Kenya faces high childhood mortality from preventable illnesses such as diarrhea, malaria, and pneumonia. Integrated community case management (iCCM) addresses this burden through community-level diagnosis, treatment, and referral. This study examined the impact of digitalization and performance management on iCCM effectiveness in Busia County using a quasi-experimental design comparing Nambale (intervention) and Teso South (control).

Results showed improved care-seeking in the intervention site, with 56% of caregivers seeking care for pneumonia on the same day of symptom onset versus 35% in the control (p = 0.031). Community health providers (CHPs) managed significantly more cases of malaria (38% vs. 16%), pneumonia (19% vs. 3.7%), and diarrhea (25% vs. 0%), reducing reliance on facilities. Only 42% of ARI cases were treated in government facilities in the intervention site compared to 62% in the control (p = 0.024). These gains were enabled by digital tools and strong performance management, which enhanced CHP effectiveness and strengthened health-seeking behavior. Findings suggest that digitalized iCCM with robust performance management improves childhood illness care and reduces health system burden in low-resource settings.
